Position Summary
The Coordinated Entry Facilitator works in a coordinated effort with the Continuum of Care (CoC) Planning Director, HMIS Director and staff, CoC-funded Programs and Non CoC-funded agencies, to manage the development, implementation, and coordination of the client referral and prioritization of care system in Memphis and Shelby County. The Coordinated Entry Facilitator is the lead contact for all coordinated entry calls and outreach facilitation. Additional responsibilities include screening clients and gathering information in order to coordinate services; facilitating the By Name List and housing placements while leading community meetings; maintaining knowledge of all services offered by homeless housing and assistance programs; assisting in strategic planning associated with the CoC and national initiatives to end homelessness as it relates to coordinated entry; tracking client referrals and outcomes in order to measure the effectiveness of the coordinated entry system, individual program outputs and regional progress towards ending homelessness; educating and communicating with community partners as it relates to the coordinated entry system.
