Position Summary
Works under the supervision of the Chief Development Officer to answer calls from seniors, caregivers, professionals and other inquirers on regarding the sate-wide CHOICES Program. The position will perform in-depth screening of callers for in-home services utilizing procedures established by the Tennessee Commission on Aging and Disability.

$2,646.00 Monthly

Minimum Qualifications
Two (2) years of direct experience assisting with referrals and client needs programs; graduation from an accredited college or university with a Bachelor’s Degree in Social Services, Nursing, Sociology or closely related field or equivalent combination of education and experience. (Experience in case management programs assisting senior citizens/persons with disabilities preferred.) (PROOF OF EDUCATION IS REQUIRED.)
Special Requirements
1. Must possess a valid Tennessee driver’s license or secure one as of employment date.
2. Must have daily access to an operate automobile and meet County requirements for automobile insurance upon date employment.
Duties and Responsibilities
1. Performs in-depth consumer screening for the CHOICES program.
2. Completes date-entry of consumer and call information into Beacon and SAMS software programs.
3. Provides comprehensive information and referral services on community programs to consumer, caregivers, and professionals, as required.
4. Conducts initial contacts to appropriate service provider agencies on consumer’s behalf when appropriate; makes follow-up contacts to determine if consumer’s needs were met and if additional referrals are necessary and/or interventions are needed.
5. Cross-trains other Community Relations Department staff to provide services to clients during all regular business hours.
6. Establishes and maintains a cooperative relationship with all local information and assistance programs, referring agencies and other social service organizations.
7. Records and analyzes caller data to be used for planning and development of future services.
8. Attends community social service functions for educational enrichment and outreach opportunities.
9. Attends professional training programs, workshops and staff meetings as required.
10. Performs other duties as assigned or directed.

1. Ability to establish and maintain effective working relationships with clients, agencies and co-workers.
2. Knowledge of office practices, procedures and equipment.
3. Ability to utilize computer and possess data entry skills.
4. Ability to plan, organize and maintain an even flow of work.
5. Ability to provide effective customer services in a fast-paced environment.
