 SUMMARY:

The Friends For Life Corporation Life Peer Mentor will serve as a primary person of contact to provide support for newly HIV infected women, men and their families and person with HIV/AIDS who have been lost from care. 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Directly interacts with and provides support for HIV/AIDS infected women, men and their families.
  • Provides mentoring services to newly diagnosed individuals and persons lost to care.
  • Provides risk reduction counseling.
  • Establishes a trusting relationship with HIV/AIDS infected/affected men and women.
  • Initiates and maintains regular contact with assigned clients.
  • Acts as an information resource to HIV/AIDS positive individuals.
  • Acts as a liaison between the Positive Living Center, Food Pantry and other community based agencies and organizations.
  • Maintains accurate, up-to-date written records of all client contracts.
  • Maintains all client related information under strict confidentiality.
  • Attends regular staff meetings.
  • Performs data entry in the CAREWare Database system.
  • Maintain all files in accordance with funding source and agency standards.
  • Make appropriate referrals for community resources for all persons supported.
  • Make appropriate referrals for Friends For Life services for all persons supported.
  • Track receipt of referrals for all persons supported.
  • Provide accurate, complete and timely data entry and reporting of all relevant program and consumer data into CAREWare, HMIS, Evaluweb and/or other reporting systems as requested by supervisor.
  • Other duties as assigned.

NON-ESSENTIAL JOB FUNCTIONS:

  • Answer incoming telephone calls on an as needed basis.

EDUCATION and/or EXPERIENCE:

  • High school diploma or GED

ESSENTIAL DUTIES AND RESPONSIBILITES

Grants and Contract Management

Research grants funding to support mission of Friends For Life. Write grant proposals to support mission of Friends For Life. Prepare monthly, quarterly and annual grant and contract reports. Oversee grant contracts to ensure timely earning of contracts. Maintain open communications with grant funders. Review all contracts for accuracy prior to execution. Maintain all grant and contract information in FR50. Complete reports as required that show annual levels of grants submitted and funded. Maintain electronic file of all grant requests, proposals and contracts and other grant/contract related materials. Other duties as assigned.

Evaluation

Take lead in ensuring agency-wide evaluation program is maintained. Ensure all program assessments are completed on schedule, data entered into CAREWare and program reports completed on schedule. Update evaluation forms as necessary. Complete all reports necessary to maintain up-to-date evaluation data. Analyze all data to ensure proposed program outcomes are being met. Provide accurate, complete and timely data entry and reporting of all relevant program and consumer data into CAREWare, HMIS, Evaluweb and/or other reporting systems as requested by supervisor.

 

ADDITIONAL DUTIES AND RESPONSIBILITIES:

Manage all aspects of the Friends For Life website. Create, write and distribute an agency e-newsletter monthly.

 

EDUCATION and/or EXPERIENCE:

Bachelor’s degree or equivalent experience required; Master’s degree preferred. Five years grant research and grant writing experience preferred. Proven success in grant writing and securing funding. Five years of experience in the development of programs to meet the needs of persons with HIV/AIDS preferred. Nonprofit background preferred. Minimum of 3-5 years management/supervisory experience preferred. Demonstrated leadership skills. Effective communication and interpersonal skills. Working knowledge of MS Windows and MS office suite with an emphasis on MS Word and MS Outlook. Strong analytical and research skills. Position Overview

The Friends For Life SWAT Team Navigator will serve as a primary person of contact to provide referrals and linkages between African American, Hispanic/Latina, and other racial/ethnic minority women (ages 18 years and older), including heterosexual, lesbian, bisexual, and previously incarcerated women, as well as these women’s significant others, who have substance use or co-occurring substance use and mental disorders, and are living with or at risk for HIV/AID seeking services at Friends For Life to intensive outpatient alcohol and drug treatment services. This position will also be responsible for providing HCV and HIV testing and linkages to the appropriate medical care, as needed.

ESSENTIAL DUTIES AND RESPONSIBILITES.

Recruit, screen, and link women seeking services at Friends For Life to intensive outpatient alcohol and drug treatment services. Directly interacts with and provides support for identified participants. Coordinate HCV and HIV rapid testing for identified participants and their partners. Provide risk reduction counseling and evidence based interventions, including, but not limited to, SISTA, Sister to Sister, and CLEAR. Establish a trusting relationship with identified participants. Act as an information resource to identified participants. Act as a liaison between Choices, Friends For Life, and Serenity Recovery Centers, as well as other community partners as identified. Maintain accurate, up-to-date written records of all client contracts. Maintain all client related information under strict confidentiality. Attends regular staff meetings. Maintain all files in accordance with funding source and agency standards. Reports to Program Manager and Case Manager any notable changes or concerning matters. Participates in general cleaning of office and common areas. Carry out responsibilities pertaining to the efficient operation of the facility, including all events, activities, utilizing safety management, emergency procedures and non –violent crisis intervention. Monitors safety and security of environment. Reports safety and security issues to Program Manager. Manage crisis and emergency situations involving residents. Interact appropriately with any and all visitors, family members, and care providers of tenants. SUMMARYThe Friends For Life Corporation Property Maintenance Technician will be responsible for safely maintaining the physical condition and daily appearance of all agency owned and/or operated by Friends For Life.

ESSENTIAL DUTIES AND RESPONSIBILITES include the following.  Other duties may be assigned. Perform, with good workmanship, all assigned maintenance projects in a timely manner. Perform electrical repairs such as outlets, wall switches, and light fixtures, circuits, ceiling fixtures, and ceiling fans. Perform plumbing work, such as cleaning pipe clogs, replacing fittings, stopping leaks. Repairing or replacing toilets, hot water heaters, kitchen and bathroom faucets, and feed lines, P-traps, washers, seats and stems.  Perform HVAC services such as cleaning condenser units, changing filters, repairing or replacing thermostats. Repair or replace as necessary, glass, tile, carpet, fixtures, blinds, locks, doorknobs, and hinges. Paint interior surfaces as directed. Perform carpentry work as directed. Make minor concrete and masonry repairs as directed. Responsibility for proper usage of maintenance material and stock control, reporting possibility of shortages to supervisor before depletion. Carry out preventative maintenance for all agency properties according to established PM schedules. (Monthly, quarterly, semi-annual and annual).  Assist as requested with regular inspections, reporting all violations, discrepancies and damage, including those incurred in contract services, during scheduled building checks, apartment move outs and move-ins and regular work routine.  Ensure that all maintenance has been performed to prevent failure of Habitability Standards inspections or other Code inspections. Perform minor refurbishment of all apartments prior to occupancy.  Complete service repairs requested by residents of apartments.  Perform monthly plumbing inspections in all apartments.  Maintain community grounds, common areas and parking lots of all FFL properties.  Provide supervision to all outsourced contractors performing work on agency properties.  Have working knowledge of all utility and alarm cutoffs and clean outs.  Follow agency bidding protocol for work that has to be outsourced. Submit bids within 10 days of report.  Keep accurate records on maintenance request forms noting date and time completed and any unusual incidents regarding maintenance requests.  Submit on a daily basis completed maintenance request forms.  Maintain and secure all general area supply rooms.  Have proper insured transportation, available for carrying out requested work on a timely basis. Ensure compliance with standard safety programs and procedures. Insure that all agency facilities and properties are locked and secure to prevent loss of agency resources. Follow agency purchasing procedures.  Must comply with all OSHA and agency safety policies at all times.

For the 35 North Claybrook and 32 North Claybrook properties, property, the job functions include:  Sweep, dust, and mop downstairs and upstairs hallways and stairs. Pick up paper and other debris around property and in all common areas. Wash walls in all common areas, including doors and stair rails. Dust and remove cob webs from inside hallways and on the outside of the building. Sweep the parking lot, walkways and side walk in front and sides of building. Complete and submit maintenance requests for common areas of building (as needed) to Housing Director.  Insure that trash cans or other containers on the outside of the building that are used for cigarettes contain sand at all times. Vacuum mats in hallway area. Maintain OSHA protective standards in performing job duties. Other duties as assigned.

For the 43 North Cleveland property, the job functions include:  Empty trash containers in offices and other areas. Replace trash containers with liners.  Carry trash to outside dumpster.  Vacuum offices and hallways. Clean break room area including refrigerator and microwave. SUMMARY: The Short Term Rent Mortgage and Utility Assistance (STRMU) Case Manager will provide daily case management oversight to the tenants participating in the STRMU program in the counties of Tunica, Tate, Marshall, DeSoto in Mississippi, Shelby, Tipton, Fayette in Tennessee and Crittenden, Arkansas.

 ESSENTIAL DUTIES AND RESPONSIBILITES include the following. Other duties may be assigned.

Work in conjunction with Mississippi, Tennessee and Arkansas agencies, to get referrals for the STRMU program. Network in community to advertise availability of STRMU program. Provide education with tenant regarding eligibility requirements of STRMU program. Attend orientations, meetings with landlords, etc. with participants of the STRMU program as needed. Acquire necessary documents from landlords in order to process applications while maintaining client confidentiality. Train STRMU personnel on processing STRMU applications, including calculation of financial eligibility. Determine eligibility of persons applying for STRMU funds. Assign cases to STRMU case managers in absence of Housing Director within 24 hours of receipt of application. Staff cases with STRMU case managers to determine if application will be approved or denied according to STRMU guidelines. Sign approval or denial letters in absence of Housing Director. Facilitate inspection of and approval of housing for consumers. Determine financial eligibility of participants. Track eligibility period for each approved STRMU participant. Requisition payment for approved rent/utility assistance in timely manner. Work closely with Housing Director and residents to maintain client compliance with Lease, Service Agreement, Property Rules and subsequent agreements to reduce evictions and client decompensation. Develop Housing Service plans for all consumers participating in the STRMU program. Provide case management to housing residents and make appropriate referrals for community resources. Secure and refer participants to appropriate supportive services. Work with clients to understand and comply with Housing plans and any treatment plans. Maintain all files in accordance with funding source and agency and Housing department standards. Document all activity with STRMU participants on progress note forms. Submit Service Activity Logs on daily basis. Keep all charts up to compliance, with current case notes and appropriate forms. Generate monthly, quarterly and annual reports. Provide transportation for STRMU consumers as appropriate. Attend all trainings and meetings as assigned.

EDUCATION and/or EXPERIENCE:

Must possess a Bachelor’s degree in Behavioral Science. Must have experience working with housing programs for special needs individuals, mental health facility, homeless facility or related housing facility. Must have experience with locating housing for special needs population. Must have experience with mentally ill and or dually diagnosed population. Must have knowledge of HIV/AIDS. Position Overview

The Friends For Life Corporation Non-Medical Case Manager will ensure that consumers are provided with direct clinical support services, information, advocacy, and appropriate and necessary referrals to other applicable network providers in order to facilitate the consumers medical adherence and to empower those affected by HIV/AIDS to optimize their longevity and quality of life. The Non-Medical Case Manager coordinates care and services of consumer populations across the continuum of care network.

Essential Job Functions

Clinical duties include initial assessment, development of a Wellness Plan, and implementation of Wellness Plan through appropriate follow-up contacts. Establish and continually improve the provision of professional, comprehensive, and effective HIV services for all consumers and affected individuals. Provide leadership and direction consistent with program philosophy and policy. Meet regularly in individual and/or group supervision to: Ensure that services are being provided to meet the needs of consumers and affected individuals; Ensure that appropriate communication is occurring with agency representatives and affected individuals; Review Wellness Plan relationships and progress; Ensure that Wellness Plan steps are being implemented; Provide Case Management leadership in all matters that address HIV issues.  Manage a caseload of 50 to 80 service consumers monthly. Conduct outreach at homeless shelters, behavioral health hospitals, alcohol and drug treatment facilities, correction facilities, housing programs and other places where persons with HIV/AIDS may be receiving services.  Serve as a HIV tester as needed or directed.  Provide case management as needed in the Wellness Plan to eliminate barriers. Attend monthly meetings of the Greater Memphis Interagency Coalition for the Homeless. Attend monthly meetings of the MidSouth Coalition on HIV/AIDS. Serve on the Coalition’s Case Management Subcommittee.  Maintain ongoing contact with possible referral sources. Perform data entry in the CAREWare Database System. Perform Ryan White recertification for consumers. 24-hour on-call availability could be required. Other duties as assigned.

Preferred Requirements

Individuals with a Master’s degree in Social Work or closely related health or human services discipline with the equivalency of one year of fulltime professional case management in a public service agency serving individuals affected by HIV/AIDS preferred. Bilingual Spanish and English preferred – must be able to demonstrate fluency.

 

Minimum Requirements

Position Overview

The Friends For Life Corporation Director of Wellness Programs will serve as the key personnel responsible for overseeing the daily operations pertaining to the delivery of direct services to consumers of the agency services.  The Director of Wellness Programs is a member of the Leadership Team and is responsible for ensuring that clinical services meet clinical outcome measurements.

 

Essential Job Functions

Oversee all departmental functions of the wellness program. Serve as the direct supervisor of the Wellness Program staff. Ensure all wellness departmental outcome and programmatic requirements are being met for each program. Develop, implement, provide, and supervise staff for consumer advocacy, supportive services including wellness, nutritional, and prevention education while serving individuals affected by HIV/AIDS. Work with Friends For Life staff to identify and solicit local, state and/or federal grants which support and enhance the capacity of clinical and supportive services. Participate in all applicable community organizations, coalitions, and Continuum of Care groups toward strengthening partnerships which will benefit service to consumers. Ensure staff and programmatic adherence to organizational policies and procedures.  Routinely review, revise, and create organizational policies and procedures as are applicable. Complete routine quality audits to ensure timeliness and accuracy of staff completion of CAREWare data. Complete routine quality control and audits of clinical services’ records. Ensure the maintenance of confidential records in keeping with all applicable ethical guidelines and legal requirements.  Ensure staff adherence to all applicable ethical guidelines and legal requirements. Responsible for a developing, maintaining, and monitoring a clearly communicated due process procedure for addressing consumer complaints. Provide ongoing training for wellness program employees. Work with appropriate staff and consultants to maintain an evaluation program for all services. Monitor program budgets. Assist in the development of new programs to meet emerging needs. Oversee all activities of the Wellness University (WU) and the Positive Living Center (PLC), Serve as the Chair of the Annual Wellness Retreat. Market all components of the Wellness Program and the Positive Living Center to potential clients/students and potential referral sources. Oversee the development or coordination of Wellness Plan for each program participant Oversee the monitoring of the implementation of each participant’s Wellness Plan.  Oversee the development and maintenance of written documentation of the curriculum for the Wellness University.  Oversee all aspects of the Kroger Pharmacy program, nutritional services, prevention education, HIV testing, and consumer advocacy. Ensure monthly schedule of all activities is completed by beginning of last week of each month. Monitor the daily documentation of all activities within the Wellness Program. Develop and maintain written Standard Operating Procedures for all Wellness Programs. Position Overview

The Friends For Life Housing Administrative Assistant will provide administrative support to the Housing Department as assigned.

Essential Components

Shelter Plus Care Program (S+C), Tenant Based Rental Assistance Program (TBRA), Short Term Rent, Mortgage and Utility Assistance (STRMU), Representative Payee (RP)

Essential Job Functions

Provide timely clerical support and program assistance to the Housing Department as assigned by the Director of Housing Operations.  Schedule and conduct inspections of properties within the Housing Department in conjunction with the Director of Housing Operations.  Notify tenants in writing, at least 24 hours in advance, of property inspections.  Maintain Housing Department files as directed.  Assist in monthly, quarterly and or annual review of Housing charts.  Document all maintenance requests in maintenance log, submit requests to Property Technician and copy to Director of Housing Operations.  File all completed maintenance request work orders.  Maintain maintenance request log and requests in orderly and up to date manner. Print out , submit and track monthly preventative maintenance requests. Maintain keys to all agency properties in secure lock boxes.  Use sign in/out sheet to track distribution/check out of any agency keys.  Coordinate with Housing Director as assigned, the scheduling of vendors for routing property services such as pest control, lawn care, etc. for agency properties. Compile and distribute monthly rent statements for Housing tenants by the 20th of each month. Receive and receipt rents for Housing tenants by the 5th of each month.  Track late rents, compile and mail late rent statements by the 6thth of each month.  Direct all receipts and transactions to the business office according to the SOP for rent collection.  Submit applicable subsidy report by the 5th of each month.  Submit applicable match report by the 5th of each month. Submit additional reports according to schedule as assigned.  Track recertification dates and mail recertification letters for S+C according to HUD timelines. Typing as assigned. Enter Surveys/Program Assessment data into CAREWARE weekly. Maintain Service Activity Log (SAL) binder on a daily basis. File progress notes, other documents submitted for housing program charts. Maintain all Housing program waiting lists, according to HUD guidelines. Issue Representative Payee checks and maintain proper documentation of check issuances.  Complete Service Activity Log (SAL) each time that representative payee activity is conducted.  Monitor supplies for Housing Department to prevent depletion of supplies.  Submit purchase orders to request additional supplies. Route client walk–in inquiries to appropriate housing personnel. Maintain and distribute Housing Department numbered request for assistance forms.  Issue vendor checks as requested. Issue bus tokens to Housing tenants as needed. Receive incoming orders for janitorial and office supplies in absence of environmental tech and property maintenance tech. Collect supportive services forms monthly for match report. Submit report of outstanding maintenance requests monthly. Mail maintenance surveys to Housing tenants within 48 hour of completed maintenance, track and file returned surveys.

Requirements

Must have a high school diploma or GED equivalent. Must have intermediate to advanced computer literacy and skill level.  Must have ability to create documents in Excel.  Must have excellent organizational and recordkeeping skills. Must be able to multitask in a fast pace environment.  Must be able to complete assignments with accuracy and in a timely manner with minimal direct supervision.  Must be able to work well with all types of people.  Regular and predictable attendance is necessary.  Maintain confidential information and adhere to all policies and procedures regarding Protected Health Information(PHI)

Position Overview

The Friends For Life Corporation Clinical Services Liaison will greet visitors/telephone callers pleasantly & professionally and to convey a feeling of warmth, respect & understanding.  Will occasionally be required to type reports and letters, make copies, and perform other administrative and clerical duties as assigned.

 

Essential Job Functions

Greet visitors, clients, and vendors to the agency and direct them to the appropriate staff person.  Answer the telephone and direct callers to the appropriate staff person, or take messages when necessary.  Maintain consumer files.  Develop appointment system, and set appointments for various programs.  Develop and maintain appropriate database systems (tracking, appointments, etc.).  Develop and maintain client referral system. Monitor IT support Desk and coordinate with IT vendor.

 

Non-essential Job Functions

Place telephone calls when directed to do so.  Type reports, letters, and forms and address envelopes, and make copies when necessary.  Use the postage meter and fax machine when needed.  Receive in-kind donations to the agency and record all gifts.  Accept donations and disperse donation receipts.  Assist with typing articles for the newsletter.  Provide clerical and administrative support to various program areas as directed and needed.  Maintain staff directory and phone extension list.  Other duties as assigned.

 

Requirements

High School Diploma with some office and business emphasis, or possess demonstrated experience in a similar manner.  Database development and management required.  Must have IT problem solving skills.  Must possess good interpersonal and communications skills, with emphasis on telephone courtesy.  Ability to greet visitors, clients, and vendors to the agency with courtesy, professionalism, and tact.  Must be sensitive to the culturally and racially diverse nature of clients and visitors to FFL.  Must be able to interact with everyone in a friendly, helpful manner.  Must possess basic PC skills.  MS Word, MS Excel, along with typing skills.  Must understand the confidential nature of all reports, messages, and clients that he/she comes in contact with respects that privacy at all times, both at work and away from work.  Two years’ experience working with persons with HIV/AIDS preferred.  Regular and predictable attendance is necessary.  Maintain confidential information and adhere to all policies and procedures regarding Protected Health Information(PHI).

Position Overview

The Aloysius New Beginnings (ANB) Supportive Housing Program Case Manager will provide daily case management oversight to the tenants participating in and housed in the Supportive Housing Program (Aloysius New Beginnings) owned and operated by Friends For Life Corporation.

Essential Job Functions

In conjunction with the Program Manager, locate appropriate candidates for the Supportive Housing program (SHP).  Locate appropriate and affordable housing for persons moving out of the SHP program.  Work closely with Program Manager and residents of the SHP program to maintain client compliance with Lease, Service Agreement, Property Rules and subsequent agreements to reduce evictions and client de compensation.  Develop Housing Service plans for all consumers entering the Supportive Housing program.  Work with clients to understand and comply with Housing plans and any treatment plans.

Advocate client issues to create a network of understanding and trust for both resident and service provider and to identify potential problems for the client and to communicate those problems to the Program Manager. Research and find available resources necessary for tenants to maintain housing, health, employment, etc. Conduct weekly inspections of apartments for the purpose of assisting tenants with any issues with housekeeping.  Provide housekeeping assistance and or resources necessary for tenant to maintain cleanliness of apartments.  Develop budgets with clients. Monitor compliance with health services. Work with community Case managers to educate regarding the eligibility and application process for SHP program.  Provide case management to housing residents and make appropriate referrals for community resources.  Participate in interview process for eligible applicants to fill vacancies.  Enter data into the Service Point HMIS system.  Track supportive services received by residents. Assist program manager to organize and keep turn away data on all individuals for SHP program.  Complete admission paperwork with incoming residents.  Maintain all charts according to funding sources, Housing department and agency standards.

Submit documentation of client activity on daily progress notes.  Submit Service Activity log on daily basis. Attend monthly service provider meeting for Community Alliance for the Homeless.  Attend HIV case management meetings. Attend Creative Homes Initiative meetings.  Attend all assigned meetings.  Rotate supervision of Tenant Household supply closet. Provide transportation for SHP clients as appropriate.  Coordinate tenant meetings with SHP tenants and Program manager.

Participate in orientation for all new SHP tenants prior to move-in. Develop and conduct Life Skills curriculum for all SHP tenants. Conduct weekly home visits with all SHP tenants. Insure that all tenants are submitting weekly Supportive Services records for documentation. Request and participate in staffing with Program manager and Director of Housing when tenants are non –compliant.

Requirements

Must have Bachelor’s degree in Behavioral Science.  Must have experience working with transitional housing programs or permanent housing programs for special needs individuals, mental health facility, homeless facility or related housing facility.  Must have experience with dually diagnosed population, including substance abuse and mental illness.  Must have knowledge of HIV/AIDS.  Must have computer skills in Window, Word, Excel, Access and Internet.  Must have personally owned transportation.  Must have liability insurance. Must have good organizational skills. Must have good time management skills. Must be able to complete assignments with little direct supervision. Must be able to work well with all types of people.
