Agape, a non-profit child and family services agency is a seeking dynamic, motivated and assertive ASD Connector.

Agape is seeking applicants for ASD Connector positions to effectively provide grassroots outreach and engagement of the youth and facilitate connection with them to the high quality services needed to succeed.

The goal of ASD Connector position is to be embedded in a selected site/neighborhood in Memphis with the hope of connecting with families, and assisting by linking them to resources, that they desire and need. The ASD Connector is required to go where the people are and build relationships. They must be able to focus on family strengths and recognize the family relationship as primary and essential.

They are responsible for ensuring the health and welfare of children and families served, which includes seeing that needs are being appropriately addressed, i.e., medical, health, spiritual, emotional, social, employment, educational/training, etc.

Additionally, the ASD Connector should have excellent interpersonal skills, knowledge of social services systems, knowledge of resources, good organizational, time management and communication skills.

They must possess the ability to work well independently as well as part of a team; works in partnerships and relationships with key community leaders, supporters and advocates. ASD Connectors should ensure program quality, transparency, and accurate reporting.

Education/Degree Requirement:
Bachelor Degree in Social Work or other related social services fields acceptable. Peer-based/community- based applicants with a High School degree or GED, with similar life experiences will be considered as well for this position.

Candidate must be culturally sensitive with a passion for helping others.

Must be computer literate.

Must have a valid driver license and own a vehicle with proof of auto insurance.

Our mission is to provide one location that effectively combines civil, criminal, health and social services for victims of family violence. The Center Navigator is responsible for assisting the client in accessing services from agency partners that accommodate the individual needs of the client.

The Children’s Aid Society has opening for Social Work Advisor

Job Description 

The Children’s Aid Society is a nonprofit organization that helps children in poverty to succeed and thrive.  While the majority of Children’s Aid programs are located in New York City the organization operates model initiatives outside the city’s boundaries. Currently, Children’s Aid is overseeing a program called Family Rewards, which is being implemented both in New York City and Memphis.  The program financially rewards parents and children when they complete certain activities such as attending an annual wellness visit or getting good grades in school.  

Children’s Aid is seeking a Social Work Advisor for its program team in Memphis. The Family Rewards Social Work Advisor is a full time position that will report directly to the Family Rewards Memphis Senior Program Associate. The Social Work Advisor will work closely with staff at the two local nonprofit partners (NPOs) delivering services to families enrolled in the Memphis program. The primary objective of this position is to assist NPOs in incorporating techniques and best practice related to family engagement as well serve as a professional development resource for existing staff. Specific job responsibilities are detailed below.

Primary Job Functions:

  • Work in partnership with the NPOs to develop strategies for fully engaging all members of the family including teens.
  • Assist NPOs in developing interviewing strategies integrating approaches such as motivational interviewing.

  • Conduct family observations and joint advisements in partnership with NPO staff.
  • Assist NPO staff in identifying community resources and organizational referrals.

Education and Work Requirements

  • Masters in Social Work
  • 3-5 years of relevant work experience 

Qualifications

  • Experience conducting training and technical assistance
  • Knowledge and understanding of issues facing individuals and families receiving public benefits such as cash assistance, food stamps or housing subsidies.
  • Direct service experience counseling high school students or experience in family counseling/conflict resolution
  • Experience providing college readiness support and post-secondary planning for students along the spectrum of academic performance.
  • Experience in motivational interviewing
  • Working knowledge of local social service providers and community resources
  • Well-developed communication skills (written and verbal)
  • Strong analytical skills
  • Ability to work collaboratively
  • General understanding of computer/web applications

Position Overview

The Friends For Life Corporation Non-Medical Case Manager will ensure that consumers are provided with direct clinical support services, information, advocacy, and appropriate and necessary referrals to other applicable network providers in order to facilitate the consumers medical adherence and to empower those affected by HIV/AIDS to optimize their longevity and quality of life. The Non-Medical Case Manager coordinates care and services of consumer populations across the continuum of care network.

Essential Job Functions

Clinical duties include initial assessment, development of a Wellness Plan, and implementation of Wellness Plan through appropriate follow-up contacts. Establish and continually improve the provision of professional, comprehensive, and effective HIV services for all consumers and affected individuals. Provide leadership and direction consistent with program philosophy and policy. Meet regularly in individual and/or group supervision to: Ensure that services are being provided to meet the needs of consumers and affected individuals; Ensure that appropriate communication is occurring with agency representatives and affected individuals; Review Wellness Plan relationships and progress; Ensure that Wellness Plan steps are being implemented; Provide Case Management leadership in all matters that address HIV issues.  Manage a caseload of 50 to 80 service consumers monthly. Conduct outreach at homeless shelters, behavioral health hospitals, alcohol and drug treatment facilities, correction facilities, housing programs and other places where persons with HIV/AIDS may be receiving services.  Serve as a HIV tester as needed or directed.  Provide case management as needed in the Wellness Plan to eliminate barriers. Attend monthly meetings of the Greater Memphis Interagency Coalition for the Homeless. Attend monthly meetings of the MidSouth Coalition on HIV/AIDS. Serve on the Coalition’s Case Management Subcommittee.  Maintain ongoing contact with possible referral sources. Perform data entry in the CAREWare Database System. Perform Ryan White recertification for consumers. 24-hour on-call availability could be required. Other duties as assigned.

Non-essential Job Functions

Represent the Agency at outreach fairs as needed or directed. Assist in planning and developing programs and services.

Preferred Requirements

Individuals with a Master’s degree in Social Work or closely related health or human services discipline with the equivalency of one year of fulltime professional case management in a public service agency serving individuals affected by HIV/AIDS preferred. Bilingual Spanish and English preferred – must be able to demonstrate fluency.

 

Minimum Requirements

A Bachelor level degree in Social Work and equivalency of one year of full time professional case management in a public service agency. – or – A Bachelor level degree in a health or human services related discipline with the equivalency of two years full time professional case management in a public service agency. Two years experience working with persons with HIV/AIDS preferred. Demonstrated fluency in Spanish and English is preferred.  Must have own transportation. Excellent communication skills. Excellent negotiation skills. An ability to work well with all types of people.  Intermediate to advanced skills in Microsoft Office Suite.  Regular and predictable attendance is necessary.  Maintain confidential information and adhere to all policies and procedures regarding Protected Health Information(PHI).

 

Other Skills/Abilities.

The ability to work well in a fast paced environment.

Physical Demands

Extended periods of standing and sitting. The ability to lift 20 -25 lbs as needed.

 Work Skills

In the office setting supervisory experience is needed along with staff training. Need to have a working knowledge of community resources and disability services available inTennessee for children and adults with disabilities as well as knowledge of government systems such as TennCare, Social Security Disability, Individuals with Disabilities Education Act (IDEA), and Americans with Disabilities Act (ADA).
